Simple Prescription is a visual prescription template developed by Escola de Pacientes and adapted to the needs of people with chronic diseases. With clear language, pictograms, and organization by times of day, the instrument makes prescriptions easier for patients and pharmacists to understand, reducing medication-use errors in Primary Health Care.
The project was born in 2016-2017 at UBS 2 in Itapoã (SES-DF), was scientifically validated by the Delphi method with Primary Care professionals and, after years of use and refinement, was institutionally adopted by the Federal District State Health Department.
